We specialize in helping older adults stay safe, active, and independent with personalized care, engaging programs, and a team that truly understands your needs. LHANC has served as a TPA-licensed vendor for Sunshine Health in Miami-Dade, Broward, and Palm Beach County since 2017.

INTERESTED IN BECOMING A LHANC MEMBER?
Long-Term Care enrollees must be eligible with Sunshine Health to receive LTC services through Little Havana Activities & Nutrition Centers of Dade County, Inc. ("LHANC").

For LHANC enrollment, members or approved family members must request a change in case management to LHANC / FCMSA by contacting their assigned Sunshine Health case manager.
